Severely disabled person's pass

You can obtain an application for a severely disabled person's pass from the Citizens' Office.
Even if you already have a severely disabled person's pass, you can obtain a so-called amendment application from us. Please complete the application and hand it in to us.
We will send it to the social welfare office of the district of Warendorf. The district of Warendorf will then contact your doctors to check the application requirements.
